5 Common Causes of Shoulder Pain

Your shoulders play a role in almost every upper-body movement, from lifting and reaching to throwing and pushing.
Because of that, shoulder pain can quickly affect work, sport, and daily life.

1. Rotator Cuff Strain or Tendinopathy

The rotator cuff is a group of muscles that stabilise your shoulder joint.
Overuse, repetitive lifting, or poor technique can irritate these tendons, leading to pain when raising your arm or lying on the affected side.

Symptoms often include dull aching, weakness, or stiffness. Early management and guided loading exercises are key to recovery.

2. Shoulder Impingement

Shoulder impingement occurs when soft tissue structures become compressed as you lift your arm.
This is often due to postural changes, muscular imbalance, or inflammation.
It can cause sharp pain when reaching overhead or behind your back.

Improving shoulder control, posture, and mobility helps reduce irritation and restore pain-free movement.

3. Poor Posture or Muscle Imbalance

Hours spent sitting or working at a desk can cause the chest muscles to tighten and upper back muscles to weaken.
This forward-shoulder posture increases stress on the shoulder joints, often leading to discomfort or fatigue during activity.

Targeted strengthening and mobility work can help rebalance these muscles and relieve pressure.

4. Bursitis or Inflammation

The shoulder has small fluid-filled sacs (bursae) that cushion movement.
When these become irritated, swelling and inflammation can limit range and cause tenderness.

Manual therapy, rest from aggravating movements, and gradual reloading can help reduce symptoms.

5. Referred Pain from the Neck or Upper Back

Sometimes shoulder pain isn’t coming from the shoulder at all.
Tight or restricted joints in the neck or upper back can refer pain into the shoulder region.
Osteopaths assess the whole upper body to identify whether neck function or rib mobility is contributing.
